Obtaining your Medical Records
How to request and obtain your medical records:
Before your doctor evaluation appointment, it's imperative that you obtain a copy of your medical records, indicating your diagnosis in order to get medical marijuana in any state.
Step 1: Contact your primary care doctor
Most practices require you to fill out a specific form to formally request your
medical records. To obtain this form, simply call your primary care doctor's office,
or the office of the doctor who gave you the diagnosis, and request a copy. They should be able to deliver it to you by fax, email, postal mail or you may pick it up from the doctor's office.
If the doctor's office doesn't have a specific form, simply write a letter to request your records. Be sure to include:
Your Name
Social Security number
Date of Birth
Address
Phone Number
Email Address
Record(s) being requested
Dates you were under the doctor's care
Signature
Delivery option (pick up, fax, email, etc.)
Your medical records need to be in the AZCU Office before the day of Doc. Apt.
If you suffer from one of the debilitating illnesses identified by the state and believe that medically recommended cannabis can be an effective element in your treatment, consult with your regular physician.
If you qualify, the recommending physician will provide you with a signed Arizona Department of Health Services physician certification form.
Arizona drivers license issued on or after October 1, 1996; OR
Arizona identification card issued on or after October 1, 1996; OR
Arizona registry identification card; OR
Photograph page in the patients U.S. passport; OR
An Arizona drivers license or identification card issued before October 1, 1996 AND one of the following:
Birth certificate verifying U.S. citizenship
U.S. Certificate of Naturalization
U.S. Certificate of Citizenship
A current photograph of the qualifying patient taken no more than 60 days before submission of the application. Must be in natural color, front view of the individuals full face, without a hat or headgear that obscures the hair or hairline, with a plain white or off-white background.
Payment of $150.00 to the State of Arizona Health Dept. Unless you have the SNAP card (food stamps), then the fee would be $75.00 to the State of Arizona Health Dept. Must bring in your SNAP card to get the Discount. An email address is required as the State will email the approval or denial letter. If approved, you will receive the medical marijuana card within 14 days. Once you have received the email from the State verifying that you have been approved you may legally obtain medicine.
